Administered by Home Care Partners and Rebuilding Together, both DACL grantees, Safe at Home provides preventative adaptations and evidence-based classes and other activities to reduce the risk of falls. Safe at Home helps older adults and residents with disabilities to age in place. It provides preventative home modifications to reduce the risk of falls in the homes of qualifying seniors age 60+ and adults with disabilities age 18-59. DC residents approved for the program receive up to $7,000 of home modifications. Home modifications are completed by licensed contractors vetted through a competitive bidding process. Modifications include stair lifts, grab bars, railings, raised toilet seats, ramps, etc. Modifications are tailored to participant needs based on in-home assessment conducted by Occupational Therapists with an Executive Certificate in Home Modifications and Certified Aging in Place certifications.
